Duties may include:

  • Personal Care Needs: Assisting with bathing, dressing, grooming, and restroom use.
  • Mobility Assistance: Helping with transfers, walking, and range of motion exercises.
  • Daily Living Assistance: Supporting meal preparation, feeding, and light housekeeping.
  • Companionship: Engaging with clients to reduce isolation and supervising activities.
  • Medication Supervision: Following a dosing schedule and ensuring medication self-administration.
  • Respite Care: Providing relief for primary caregivers in home settings, healthcare facilities, or adult daycare centers.

Minimum Qualifications:

  • Education: Have a high school diploma or equivalent
  • Training: Complete a first aid course and cardiopulmonary resuscitation (CPR) training within the last year
  • Background check: Pass a background check, such as a live scan
  • Experience: Have at least one year of experience working with children or adults with disabilities
  • Driver’s license: Have a valid California driver’s license
  • References: Be able to provide two references
  • Compliance: Meet the training and agency compliance requirements for the position

Home Care Coordinator

As a Home Care Coordinator you will play a crucial role in managing and organizing care for individuals in their homes. You will be responsible for coordinating daily activities, such as scheduling medical appointments and ensuring timely doctor visits. Additionally, you will maintain accurate records of end-to-end patient care management processes and document signs and symptoms in electronic systems. You will also provide oversight for medication administration, ensuring individuals receive all prescribed medication according to protocols. Furthermore, they work with multiple disciplines, including social services, occupational therapy, speech therapy, nursing, and physicians, to ensure care complies with all relevant policies, procedures, and regulations.

Other Responsibilities include:

  • Manage social media accounts such as Facebook and Instagram to generate new clientele.
  • Manage caseload by providing care coordination to individuals with a primary intellectual/developmental disability diagnosis all of whom receive community-based waiver servicess
  • Verify daily duty logs for completion and accuracy according to HIPAA standards.
  • Maintain monthly home care reports and update client service records as needed in CMS.
  • Assist in obtaining and processing physician orders and documents require according to CMS guidelines.
  • Maintain medical records, file HMO/Medicare budget approvals in client records, follow HIPAA guidelines.
  • Coordinate daily activities including: CPR instruction, apnea monitor training and in home follow-up after discharge.
  • Provide assistance with personal hygiene, physical comfort, nutrition, prevention of skin breakdown, rehabilitation and safety.
  • Provide strength-base, culturally sensitive, community-base in-home therapy to families and individuals, including adolescents and children.
  • Provide direct service to the developmentally disabled residents, including intervention, behavioral management and implementation of individual rehabilitation plans.
  • Examine clients arriving acutely in the ICU and administer therapeutic activities including ambulation, bed mobility, transfers and safety.

Minimum Qualifications:

  • Education: A high school diploma or equivalent is required. An associate’s or bachelor’s degree in healthcare, social work, or a related field is a plus.
  • Experience: Experience in home care or healthcare settings is a plus. This may include roles in caregiving, case management, or health services administration.
  • Certifications: Not mandatory, certifications such as Certified Home Care Administrator (CHCA) or similar credentials can be beneficial.
  • Skills: Strong organizational, communication, and interpersonal skills are essential. Knowledge of healthcare regulations and patient care practices is also important.
  • CPR/First Aid Certification: Some employers may require current CPR and First Aid certification.
